We handle minor office surgery as well, including the removal of cysts, moles, warts, and the closing of lacerations. If the need for more specialized medical care arises, our medical colleagues at Lansdale Hospital/Abington Jefferson Health stand ready to serve us. By consulting with them, we are able to offer you expertise in a medical specialty relevant to your condition. Patients now have a single, integrated medical record throughout the Jefferson system: One Patient, One Record, One Patient Portal. You can also download the app on either your iPhone or Android device. During the setup process, link your account to "Jefferson Health" and accept the option to turn on notifications, so that you never miss an important message or reminder.
Prior to your next visit, log into MyJeffersonHealth to confirm your appointment and to complete your electronic check-in. This process will allow you to update important details about your medical history and demographic information.
Green and Seidner Family Practice Associates, although an independent practice, has joined in with Jefferson's Patient Portal for all that it has to offer in addition to the importance of being able to access all of your health needs in just one portal. It is another way we aim to make it easier for you to manage your healthcare services.
Refill requests should come directly from your pharmacy. Please call your pharmacy and ask that they submit an electronic request, or you may process your refill using your patient portal account https://my.jeffersonhealth.org/
Submit your request early in the day, and early in the week. Requests received after 3:00pm will not be addressed until the next business day, and may take up to three (3) business days to complete request.
Local pharmacy: Please call the pharmacy to make sure the drugs are ready for pick-up.
Please remember to mention any prescriptions needed at the time of your office visit. We can take care of them very easily at that time.
If you had labs or imaging ordered by of office, you should expect to hear the results within one week of having the test completed. Your results will be posted to the patient portal as soon as they are available. If you are not registered on the patient portal, you will receive a call from our office staff or your provider to review your results.

Our goal has always been to provide you with the highest quality of care. Through Primary Care First, we will receive additional resources from Medicare and other health insurance companies to help us enhance our work and provide you with the best quality, patient-centered care.

As part of this program, Medicare will start sharing some of your personal health information with us, such as when you receive care at hospitals, emergency departments, and specialist offices. This will help provide us with a more complete picture of your health and allow us to better coordinate your care.

Your Medicare benefits are not changing, and this program is only intended to enhance the healthcare you receive at Green and Seidner Family Practice. You still have the right to use or visit any doctor or hospital that accepts Medicare, at any time. Your doctor may continue to recommend that you see particular doctors for your specific health needs, but it is always your choice which doctors you use or hospital you visit.

What is “balance billing” (sometimes called “surprise billing”)?
When you see a doctor or other health care provider, you may owe certain out-of-pocket costs, such as a copayment, coinsurance, and/or a deductible. You may have other costs or have to pay the entire bill if you see a provider or visit a health care facility that isn’t in your health plan’s network.
“Out-of-network” describes providers and facilities that haven’t signed a contract with your health plan. Out-of-network providers may be permitted to bill you for the difference between what your plan agreed to pay and the full amount charged for a service. This is called “balance billing.” This amount is likely more than in-network costs for the same service and might not count toward your annual out-of-pocket limit.
“Surprise billing” is an unexpected balance bill. This can happen when you can’t control who is involved in your care—like when you have an emergency or when you schedule a visit at an in-network facility but are unexpectedly treated by an out-of-network provider.
You are protected from balance billing for:
Emergency services- If you have an emergency medical condition and get emergency services from an out-of-network provider or facility, the most the provider or facility may bill you is your plan’s in-network cost-sharing amount (such as copayments and coinsurance). You can’t be balance billed for these emergency services. This includes services you may get after you’re in stable condition unless you give written consent and give up your protections not to be balanced billed for these post-stabilization services.
Certain services at an in-network hospital or ambulatory surgical center- When you get services from an in-network hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers may bill you is your plan’s in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can’t balance bill you and may not ask you to give up your protections not to be balance billed.
If you get other services at these in-network facilities, out-of-network providers can’t balance bill you, unless you give written consent and give up your protections. You’re never required to give up your protections from balance billing. You also aren’t required to get care out-of-network. You can choose a provider or facility in your plan’s network.
When balance billing isn’t allowed, you also have the following protections:
* You are only responsible for paying your share of the cost (like the copayments, coinsurance, and deductibles that you would pay if the provider or facility was in-network). Your health plan will pay out-of-network providers and facilities directly
* Your health plan generally must:
o Cover emergency services without requiring you to get approval for services in advance (prior authorization).
o Cover emergency services by out-of-network providers.
o Base what you owe the provider or facility (cost-sharing) on what it would pay an in-network provider or facility and show that amount in your explanation of benefits.
o Count any amount you pay for emergency services or out-of-network services toward your deductible and out-of-pocket limit.
